ABSTRACT

Chest trauma is one of the most common injuries. Venous thromboembolism (VTE) as a common complication of chest trauma seriously affects the quality of patients′ life and even leads to death. Although there are some consensus and guidelines on the prevention and treatment of VTE at home and abroad, the current literatures lack specificity considering the diagnosis, treatment and prevention of VTE in patients with chest trauma have their own characteristics, especially for those with blunt trauma. Accordingly, China Chest Injury Research Society and editorial board of Chinese Journal of Traumatology organized relevant domestic experts to jointly formulate the Chinese expert consensus on the diagnosis, treatment and prevention of chest trauma venous thromboembolism associated with chest trauma (2022 version). This consensus provides expert recommendations of different levels as academic guidance in terms of the characteristics, clinical manifestations, risk assessment, diagnosis, treatment, and prevention of chest trauma-related VTE, so as to offer a reference for clinical application.

ABSTRACT

Objective: To investigate the expression pattern and clinical significance of long non-coding RNA deleted in lymphocytic leu-kemia 1 (DLEU1) in non-small cell lung cancer (NSCLC), and to further evaluate its effect on tumor metastasis. Methods: Paired cancer and adjacent tissues were obtained from 42 patients with NSCLC that underwent surgical resection of cancer in The Third Xiangya Hos-pital of Central South University from January 2008 to December 2012, at the Department of Cardiothoracic Surgery. The expression features of DLEU1 in NSCLC tissue were detected by real-time quantitative polymerase chain reaction assay. Statistical methods were used to analyze the relationship between the expression of DLEU1 and the clinicopathological features and the survival time of pa-tients with NSCLC. Effects of DLEU1 on migration and invasion of NSCLC cells were evaluated by in vitro wound healing and cell inva-sion assays, respectively. Western blot was performed to investigate the protein levels of E-cadherin, N-cadherin, and Vimentin, which are the biomarkers of epithelial to mesenchymal transition (EMT). Results: Of the 42 NSCLC samples, DLEU1 expression was up-regulat-ed in 35 samples (83.33%) and down-regulated in 7 samples (16.67%). The expression level of DLEU1 in NSCLC tissue was 2.11 times that in the adjacent tissues (P<0.05). The expression of DLEU1 in four NSCLC cell lines (A549, H1299, SPCA1, and H358) was higher than that in normal lung 16HBE epithelial cells, and of these, A549 cells had the highest DLEU1 expression (P<0.05). Highly expressed DLEU1 was positively correlated with lymph-node metastasis (P<0.05), but was not significantly associated with other parameters, in-cluding patient gender, age, smoking history, primary tumor size, histological grade, and the TNM stage. The survival time of patients with high DLEU1 expression was significantly shorter than that of patients with low DLEU1 expression (P<0.05). In vitro interference with DLEU1 expression significantly inhibited the migration and invasion of A549 and SPCA1 cells compared with that of the control group (P<0.05). Upon interference with DLEU1 expression , protein levels of E-cadherin increased, whereas those of N-cadherin and Vi-mentin decreased in A549 cells in comparison with the control group. Conclusions: The expression of DLEU1 is up-regulated in NSCLC tissues and cell lines, correlates with lymph-node metastasis and survival times in patients with NSCLC, and promotes tumor cell migra-tion and invasion by regulating EMT, suggesting that DLEU1 may be a potential therapeutic target for NSCLC.

ABSTRACT

OBJECTIVE@#To investigate the protective effect of combined administration of verapamil and nitroglycerin on the function of canine transplanted lungs.@*METHODS@#Twenty orthotopic left lung transplantations were performed in 40 canines, which were randomly divided into 4 groups. In group I (control), the donor lungs were perfused and preserved with LPD solution, while group II with LPD solution plus verapamil 0.1 g/L, group III with LPD solution plus nitroglycerin 0.1g/L, and group IV with LPD solution plus verapamil 0.1 g/L and nitroglycerin 0.1 g/L. Hemodynamics and graft gas exchange were assessed 0, 2 and 4 h after the operation. The lung grafts were harvested to measure the wet/dry weight ratio, malondialdehyde (MDA) contents and superoxide dismutase (SOD) activity.@*RESULTS@#Compared with group I, II and III, the mean pulmonary artery pressure (MPAP), pulmonary vascular resistance index (PVRI), partial pressure of oxygen in arterial blood (PaO₂), dynamic compliance (Cdyn) and alveolar-arterial oxygen tension volume [P(A- a)O₂] in group IV were improved significantly (P0.05). In group IV, the wet/dry weight ratio and MDA contents were lower than those in the other 3 groups, and the SOD activity was significantly higher than that of the other 3 groups (P<0.05).@*CONCLUSION@#Verapamil and nitroglycerin in LPD solution can protect the respiratory function of canine lung grafts by attenuating pulmonary edema and oxidative stress.

ABSTRACT

Objective To review the experience of minimally invasive incision for open-heart surgery in 56 children with congenital heart diseases.Methods There were secundum atrial septal defect (ASD)25 cases, ventrilcular septal defect (VSD) 26 cases, double-chambered right ventricle (DCRV) 2 cases and pulmonary arterial stenosis (PS) 3 cases,right anterior-lateral thoracotomy for 23 cases in heart beat CPB and ministernotomy for 33 cases participated in the study.Results No death or serious complications happened,and the patients' postoperative recovery rapidly,mean hospital stay was 8 days .Conclusions Minimally invasive incision has followe as advantages,such as :less trauma, cosmetic, less pain, recovery rapid, safeness, effectiveness, and shorter hospital stay,especially for repairing the secundum atrial septal defect, membranous or para-membranous ventrilcular septal defect.

ABSTRACT

Objective To investigate the mechanism of treatment of lung cancer with Bacillus Calmette-Guerin Polysaccharides nucleic acid (BCG-PSN) combined with Cyclophosphamide(Cy). Methods 130 mice were divided into three groups (A,B and C groups). Gruop A was further divided into four subgroups . Subgroup A-1 was given normal saline orally 0.25mg/10g?d -1 ; Subgroup A-2 was given levomisole PO. 0.25mg/10g?d -1 ; Subgroup A-3 was given BCG-PSN PO.0.325mg/10g?d -1 and subgroup A-4 was given BCG-PSN by intra-abdominal injection 0.25mg/10g?d -1 . Group B was given identical drugs with group A, after fifth day given 20% sheep red blood cell 0.2ml by intra-abdominal injection for five days . Gruop C divided into five subgroups;Subgroup C1-4 were given same drugs with group A,then given 0.4% Cy by subcutaneous injection 0.25mg/10g?d -1 from fifth day to tenth day.Subgroup C-5 was normal contral group, was given normal saline 0.25mg/10g?d -1 PO and subcutaneous injection 0.25mg/10g?d -1 .Results Oral administration alone or intra-abdominal injection in subgroup A-4 obviously enhanced the phagocytosis of macrophages.The subcutaneous injection of Cy could obviously decrease anti-tumour immunity, including activity of interleukin-2 (IL-2),natural killer(NK) cell, lyphocyte trasformation, and macrophages phagocytosis. BCG-PSN could increase and rehabilitate above anti-tumour immunological function. Conclusions Combinative treatment of BCG-PSN with Cy could remarkable enhance the immunocompetence.This experiment may provide a theoretical evidence for BCG-PSN combined with chemotheapeutic agents applied to treat the lung cancer.

ABSTRACT

Objective This study is to explore the clinical experience which intravenous concentrated potassium infusion therapy to correct hypokalemia in postoperation of extracorporeal circulation.Methods the data was analyzed retrospectively on 100 consecutive patients of heart disease in Intensive Care Unit undergwent extracorporeal circulation from 1998 to 2000. A total of 219 times of infusions, with potassium concentration 112 4 to 206 mmol/L ,were administered by the central or peripheral venous route at to the rate 6 7 to 16 8 mmol/l an hour for correcting or preventing hypokalemia.Results The analysis showed that the concentration of blood potassium was safely and effectively increased without fatal arrhythmia by this way; the blood potassium was closely related to the degree of hypokalemia before operation, to the continuous time of extracorporeal circulation ,and to the urine volume in postoperation,etc,but there was no linear correlation between the increment of blood potassium and the dose of infused potassium.Conclusion It can rapidly and effectively and relative safely correct hypokalemia in postoperation of extracorporeal circulation by this way, and it has applied value in clinic .
